COR Cencora
Toll booth on U.S. drug flow; durable oligopoly, but the price already assumes most of the growth.
The business
Cencora is one of three U.S. drug wholesalers (with McKesson and Cardinal) that together move over 90% of prescription drugs from manufacturers to pharmacies, hospitals and physician practices. It earns a thin spread of about 1% operating margin on enormous volume, plus manufacturer fees and generic sourcing economics. Higher-margin adjacencies include specialty distribution to oncology and retina practices, physician-practice MSOs such as Retina Consultants of America, World Courier clinical logistics, and MWI Animal Health.

National distribution networks, DSCSA compliance infrastructure and purchasing scale make a fourth entrant uneconomic, and the three-player oligopoly has held for decades with rational pricing. Specialty and physician-services exposure adds switching costs, though customer concentration (Walgreens, Express Scripts) caps pricing power at renewals.
Steady double-digit EPS compounding, consistent buybacks that absorbed the Walgreens stake selldown, modest leverage, and disciplined bolt-ons into higher-margin specialty and MSO platforms. The RCA acquisition adds integration and physician-ownership regulatory risk, but the logic is sound.

Key risks
- Customer concentration and renewal repricing with Walgreens (now private) and Express Scripts
- Drug pricing reform (IRA negotiation, PBM reform, most-favored-nation pricing) compressing branded inflation economics or changing wholesaler fee structures
- Residual opioid litigation, Amazon or manufacturer direct-to-pharmacy disintermediation, and integration risk in physician-practice MSOs
Catalysts
- Specialty, biosimilar and GLP-1 volume growth plus MSO expansion lifting mix and margins
- Continued buybacks of about 2% of shares a year compounding per-share owner earnings
