DXCM Dexcom
Good razor-blade CGM franchise, but price bakes in ~25% growth against a durable ~10-12% ceiling.
The business
Dexcom makes continuous glucose monitoring (CGM) systems — wearable sensors, transmitters, and companion apps that give diabetics real-time blood glucose readings without fingersticks. Revenue is a razor/razorblade model: durable-ish transmitters plus a recurring stream of disposable sensors replaced every 10-15 days, sold via distributors, pharmacy, and direct channels.

Moat & management
| Moat | narrow moat |
| Source | switching costs |
| Trajectory | stable |
| Management | adequate |
Patient/physician habituation, prescriber inertia, payer formulary contracts, and integration with insulin pumps/apps create real switching costs, plus FDA approval acts as a barrier to entry. But the moat is narrower than it looks: Abbott's Libre is outselling Dexcom on volume and price, Medtronic and new entrants keep pressing, and this is a duopoly/oligopoly fight on innovation cycles (G7, Stelo) rather than a structural lock.
Capital allocation is sound — minimal dilution (share count roughly flat), reasonable balance sheet (net debt/EBITDA 0.32x), and reinvestment into R&D/manufacturing rather than financial engineering — but they've stumbled on execution (2023 guidance cut, sales-force reorg) showing the moat requires constant running to stand still.

Key risks
- Abbott Libre and other CGM competitors compressing price/share via faster iteration or non-prescription channels
- Payer/reimbursement pressure and pricing resets in international and value-based care markets
- Execution risk: past guidance misses show margin/growth is not on autopilot
- Patent cliffs or tech disruption (e.g., non-invasive glucose sensing) could leapfrog current sensor architecture
Catalysts
- Type 2 non-intensive and prediabetes market expansion (large underpenetrated population)
- International expansion and reimbursement wins
- G7/next-gen sensor accuracy and wear-time improvements widening the gap over Libre
