UHS Universal Health Services
A narrow-moat hospital operator priced for shrinkage; modest growth plus buybacks beat a pessimistic market.
The business
UHS runs acute care hospitals in growing Sun Belt markets (Las Vegas, Texas, Florida, DC area) and is the largest US operator of freestanding behavioral health facilities, plus a smaller UK behavioral business. It earns revenue from Medicare, Medicaid, managed care, and commercial payers for inpatient and outpatient care. Hospitals and psych beds are capital intensive and heavily regulated, and they depend on labor.

Local market density, certificate-of-need barriers in some states, and a leading behavioral health scale position give it real local pricing power and referral networks. But payer concentration, reliance on government reimbursement, and labor scarcity cap returns at roughly the cost of capital.
The Miller family controls the company and runs it conservatively with moderate leverage. Aggressive buybacks (shares down about 4.7% a year) at single-digit P/E multiples are value-accretive. Capex discipline is reasonable, but the company has a history of DOJ and False Claims settlements and behavioral-care litigation, including large jury verdicts.

Key risks
- Medicaid cuts and expiring ACA subsidies from the 2025 budget law hurt payer mix and supplemental payments
- Labor cost inflation and nurse shortages compress margins
- Large behavioral-health liability verdicts and regulatory or DOJ scrutiny
Catalysts
- Continued buybacks at roughly 7x earnings shrink the share count
- Sun Belt demographic volume growth and new hospital openings mature
⚠ maintenance capex ≈ min(capex, D&A), capped at FCF · Price pays for ~-1%/yr owner-earnings growth over 10y (reverse DCF); model credits 4%. · Cheap, but still in a downtrend (down (-2% vs 200-DMA, 6m -2%)) — waiting for the trend to turn before buying (don't catch a falling knife).
